Bridgemoor Transitional Care in Webster earns five stars from Centers for Medicare & Medicaid Services
CEDAR PARK, Texas—Bridgemoor Transitional Care in Webster received a Five-Star Quality Rating by the Centers for Medicare & Medicaid Services (CMS). The Five-Star Quality Rating System is used to help patients, their families and caregivers compare skilled nursing facilities based on performance in three key areas including health inspections, staffing and quality of care.

To earn this achievement, Bridgemoor Transitional Care in Webster was graded on a scale from one to five, with five being the highest, using data from surveys that occurred after March 3, 2020. The facility was judged on each of the following quality measures: health inspections, which is based on outcomes from state health inspections; staffing, which is judged on the facility’s staffing levels; and quality measures, which are calculated based on a data system that describes the quality of care provided at a facility. The scores from each category added up to an overall rating of five stars for Bridgemoor’s Webster facility.
Bridgemoor’s innovative model of care serves as a bridge between the hospital and home, helping to reduce the likelihood of a patient returning to the hospital. This practice can reduce total care costs as the patient recovers under the care of a physician-led team.
