Bridgemoor Transitional Care announces grand opening of facility in Webster
WEBSTER, Texas – Bridgemoor Transitional Care’s newly constructed, $20 million short-term rehabilitation center in Webster will open to the public on April 18, 2019. The facility will provide personalized, cost-effective transitional care following a hospital visit that enables patients to regain their highest level of independence as quickly as possible.
Bridgemoor’s model of care serves as a bridge between hospital and home, aiming to reduce the likelihood of a return to the hospital. This model preserves precious Medicare/insurance benefits through shorter lengths of stay, reducing the total cost of care as the patient recovers under the direction of a physician-led team.
“Our Webster location provides a model of care that is truly unique to the greater-Houston area,” Mark Fritz, president of Bridgemoor Transitional Care, said. “We work closely with local hospital systems, medical professionals and insurance providers to ensure an organized and smooth transition from acute care to Bridgemoor Transitional Care. Once a patient is ready to leave our facility, we work directly with the individual and their family to ensure they have the support that’s needed at home.”
Bridgemoor Transitional Care’s therapy capabilities include: physical, occupational and speech rehabilitation available onsite seven days a week; an in-house pharmacy dispensing system; dialysis coordination; electronic medical records; and other clinical support services. The rehab center can accommodate patients in 70 private suites with private bathrooms. The facility will also provide in-room and fine dining options prepared by an executive chef as well as an on-site hair salon.
The new Webster location will employ 100 full-time staff, including on-site registered nurse supervisors available 24/7, a full-time nurse practitioner, who will provide direct patient care and also educate and mentor clinical staff. An experienced medical director will oversee the coordination of medical care and treatment along with on-site specialist consultants.
